I can't tell from your problem statement if the x+5 is under the square root (radical) or just the x.  A function of x is typically denoted as f(x).

 

If the x+5 is under the radical, then it's f(x) = SQRT(x+5) = (x+5)^1/2.  The graph of f(x) will be shifted 5 places to the left along the x-axis relative to the graph of x^1/2.

 

If the x is under the radical but the 5 is not, then it's f(x) = SQRT(x) + 5 = x^1/2 + 5.  The graph of f(x) will be shifted up the y-axis by 5 relative to the graph of x^1/2
