Both gentlemen above are correct.
You can use the pt-slope formula as well.
If your point on the line is (x1, y1):
y-y1 = m (x - x1)
y = mx - mx1 + y1
Example:
Slope is 3, point on the line is (1, 2).
y - 2 = 3(x-1)
or
y = 3x - 3(1) + 2
y = 3x -3 + 2
y = 3x - 1
