H2O + H2O --> H3O+ + OH-
where the H3O+ (hydronium) has the positive charge located on the oxygen (one lone pair instead of two) and the OH- (hydroxide) has the negative charge located on the oxygen (three lone pairs instead of two). H3O+ is the conjugate acid and OH- is the conjugate base.
