Slope is rise/run or the change in y divided by the change in x,
      (y2 - y1)
m=-----------
      (x2 - x1)

      (14 -12)
m = ---------
      (6 - 4)
m = 2/2 =1 so the slope of this line is 1.

in point slope format, using slope is one and choosing the first point (4, 12)
y - 12 = 1(x -4)

to put this into y-intercept form, solve for y
a) use the distributive property :  y - 12 = x -4
b) add 12 to both sides to isolate y: y = x + 8
this is in slope intercept form: y = mx + b, with the slope = 1 and y-intercept = 8

Use algebra to put this into standard format Ax + By = C
