Initial diagnosis was wrong new diagnosis how do i code this

Question

Patient diagnosed with uti still symtomatic after 2 weeks referred to urologist that diagnosed patient with pyelonephritis. How do I code this?

You would code pyelonephritis. Since the UTI is a symptom/"part of" the reason for the underlying issue of pyelonephritis. So coding the more specified code would be appropriate here.
