Many tutors will start wit an explanation of the Guidelines. In the real world, you learn the codes as they pertain to the population your particular working situation reflects. For purposes of the certification exam, which so many employers require now, the guidelines drive your test.
The organizations assume that you can code, you would not be spending the money or taking a day of your life to prove this if the coding wasn't something you were comfortable with. So the tests are designed to demonstrate your knowledge of the guidelines.
If the challenge for you is separating the diagnoses out when there is more than one, then an example medical record would be used and some highlighting for the diagnoses that are described. Then the codes for these are secured, then the guidelines and Tabular notes are reviewed for the correct order, and any special rules.
The rest, as they say is up to the student. I hope this helps.
