Assuming meat is required and that the bread, meat and condiment are independent (that is, bread choice doesn't impact meat or condiment choice, meat choice doesn't impact bread or condiment choice, and condiment choice doesn't impact bread or meat choice):
2 bread choices x 3 meat options x 2 condiment options = 2 x 3 x 2 = 12 different sandwiches
If there's an option for no meat, we have 2 bread options, 2 condiment options and 4 meat options (ham, chicken, beef or none). This gives 2 x 2 x 4 = 16 different sandwiches.
