It means "What is your name?" indeed!
However, your usage of "e" rather than "é" brings up an important difference! In Portuguese, we actually have three forms of the letter E that mean three different things:
- é - is (this is the one you were looking for) we pronounce it like the English sound in exactly
- e - and (so technically you had written "What and your name?") we pronounce it like the English sound in ears, or the Spanish y.
- ê - the (a recent adaption in the language to address gender inclusive language)
...so what we really need here is "Qual é seu nome?"
