How to say “the Cougher”

Hello dear Hebrew speakers! I was hoping to gain some insight into a word, “cougher.” I have done a bit of ground work, and have come up “*Mishta’el*”(?), but wanted to double check. The context is that this is a nickname for a person. Any help is welcomed. Thank you!
