What you have is an equation that is in Slope Intercept Form. Your line follows the path of 1/2x+5. The +5 tells you that the line of 1/2x has shifted up 5 on the y axis. The 1/2x, tells you that the slope is 1/2, meaning it will go up one unit, for every 2 units the line moves to the right. For example, at x=0, the point on the graph would be (0,5), the next point to the right that falls on a full unit, would be at x=2, where the graph has moved 2 points to the right. If you plug in x=2, you get that y=6. Your next point would be (2,6), which is two points to the right, and one point up.
Just remember, that in Slope Intercept Form, y=mx+b, (x,y) are your points, b is your shift in the graph, and m is your slope.

You have it in the form you need. In this case, the answer is 1/2. y=mx+b. For this equation, (x,y) is always your points. b=5, and b is your shift. m=1/2, where m is slope, so your slope is 1/2. You could write it as either a sentence, or simply m=1/2.
