I am a professional software engineer and have extensive programming experience, with over two years contracting with the FAA (my current role) and stints at fortune 500 companies and startups.
My approach is to build rapport with the student, do a gap-analysis on the subject at hand and determine goals (test scores? general improvement? etc). After the gap-analysis, I ensure sessions are centered around filling the gaps by asking questions and suggesting problems that help fill the gaps. We track the concrete goals (S.M.A.R.T. based) as we go along. Once the foundational concepts are solid we steadily build from there. Sometimes, when I have bandwidth, I will write custom problem sets and we'll work through those (particularly for students falling behind).
