the vertex is (-3,-13) and the y-intercept is -4. what is f(x)?

I am having trouble doing this problem. I’m doing the correct work but seem to be getting the wrong answer every time.

I drew a sketch of the parabola and determined a third point (-6, -3).  Then I used my graphing calculator to determine the function.  I got this: y =x^2 + 6x -4.  It works for all three points.
