Slope:

m = (y1-y2)/(x1-x2)

m = (3-5)/(2-4) = (-2)/(-2) = 1

Point-slope form:

y - y1 = m(x - x1)

y - 3 = 1(x - 2)

y - 3 = x - 2

Add 3 to both sides to isolate y - leaving you with the form y = mx+b
