Currently, I use it every day at work to do data analysis in the aerospace industry. I also presented a paper on medical imaging using MATLAB at the first MATLAB user's conference in 1993.
Besides the "basic" MATLAB, I've used the signal and image processing toolboxes, the stats toolbox, the symbolic math toolbox, Simulink, the controls toolbox, and other "user contributed" toolboxes. For example, someone wrote a MATLAB library to interface to Google Earth, which I've used for work.
